It has the potential of retrograde perineural spread to the adjacent structures and spaces. Here we report a rare case of ACC of the parotid gland extended to the infratemporal fossa with perineural and vascular invasion, reflecting the advanced stage of the disease. After consulting with ENT specialists and radiation oncologists, palliative surgery was performed followed by adjuvant radio/chemo therapy.
